This website does not have an on-site contact, newsletter or medical intake form. To contact the practice, call 470-545-5551. Appointment scheduling, patient portal access and bill payment take place on external services linked from this website. Please do not send passwords, payment card details or confidential medical records through general enquiries.
3. Technical information
The website host and external resource providers may receive technical information, such as your IP address, browser information and request time, when delivering pages or fonts. This website currently has no analytics or advertising tracking integration.
4. Cookies and local storage
Your cookie preferences are saved in local storage on your device. Functional, analytics and marketing choices are optional and default to off. No tools in these optional categories are currently installed. The Cookie Policy describes this storage and how to change your preferences.
5. Patient services and external links
Our website links to NexHealth appointment scheduling, an eClinicalWorks patient portal, a payment service, Google Maps and other resources. The embedded Google Map loads automatically on pages that include it. Google may receive technical information, including your IP address, and use cookies when the map loads. Website cookie preference controls do not block this map. Starting addresses entered in the directions form are sent directly to Google when submitted. Those services have their own privacy practices and terms. Information entered there is handled by the destination service, not by this website’s cookie preference controls. Review the destination’s policies before submitting information.
6. Fonts and other resources
This website requests fonts from Google Fonts. That provider receives the network information needed to serve the fonts. Website images are served with the site. Cookie preference choices do not disable the font requests or control third-party websites.
7. Contact, retention and requests

Requests concerning patient records are handled separately from website preferences. Applicable retention requirements may affect how a request can be fulfilled.

No internet communication or electronic storage system is completely secure. Use the patient services provided by the practice for medical communications and avoid sending confidential information through general enquiries.

The website includes information about adolescent care. It does not provide an on-site account or form for children to submit personal information. Contact the office with concerns about information a child may have provided to the practice or a linked service.
